Pedal Box Access Hole?

My pedal box has a 1 5/8" access hole on top:
and going though my reference pics I find +2 with no holes, two holes:
or the fluid info plate covering the top of the box. The parts diagram shows no holes.
An open hole between the engine compartment and the interior is obviously not desirable, and have located a black plastic plug that will fit.
What the heck was original? Was it SOP to add these access holes as needed, or were they just totally random from the factory?

no holes as original. Those holes have been added by someone to allow removing master cylinders and leave pedal box in place
