Sheared Cylinder Head Bolt

My bolt is of the middle type. The unclear picture is a combination of poor light, the washer still being on and using the iphone to take the picture. I can see that this makes it look like there is no shoulder. But there is. I have now prepared the head for re-installing and am awaiting the parts to come in from RD. However, I am thinking of sending the carbs to micropolishing for overhaul now that the head is out.
Also, I have checked the torque wrench using the old bath scales trick, and it is within 2% of 70lbft, so I suspect the failure mode is an over stretched bolt due to one too many torque cycles.

My header bolt lose some of its washers and sometimes it is loosened. I am worried because it might cause me a really big problem.
