What would cause such a cyl head failure?

I don't think that area is an extension of the oil gallery to each side. It looks to be a water jacket area? The opened up area sure didn't clean up as well as the area under the cam cover. If it's water jacket, corrosion could have contributed to the damage. That casting looks pretty thin where it has been machined away for the repair. The site has it labeled as a crack. This was taken in the middle of the repair process.
At least it was fixable...
...or it could have been that whole Hadron Collider thing.
Tricky to fix and I'll bet it wasn't inexpensive.

I looked at the photos on the CTM site as well. The crack must have propagated across the top of the well between the cams. They were probably worried about making a good weld repair if the crack went all the way into the water jacket so they milled the whole area out and welded in a replacement plate. Probably a good decision. Still there is nothing offered about what caused the crack in the first place.
