Yes, done before (door card install) and looks awful IMHO. Best not to butcher the (very expensive and hence, valuable) door cards if you can help it.
In my car I have fitted speakers in the foot wells. To do this you have to cut holes in the inner panels but it's easy as they are paper thin.

The pictures are of the passenger footwell, but the driver side is in the same position. The accelerator pedal and your right foot will not catch it as there's a good gap between it and the pedal.
These are 6.5 inch Panasonic coaxial speakers but maybe for you a slightly different setup with remote tweeters mounted on the door cards or on the dash top (with double sided sticky tape) would allow you to hear it a bit better. Doesn't really matter. You're not going to hear much once you get going. For example, I have a very powerful and rather fine sounding 50Wx4 Pioneer mechless unit in my Plus 2 and the car noise easily drowns it out at speeds above 60mph.
Top tip: Use a vibrating cutting tool like a Bosch Mulitool to cut the panel. Makes very little mess and will not catch and tear the fibreglass like a reciprocating saw can.
