Fractured Clutch Disc

Just pulled the pressure plate, assuming that all I was doing was unsticking the clutch disc. A quick ten minute job, right? Wrong!
This is what I found:
Only thing I can figure, besides a defective plate, is that on some previous "unsticking" the missing portion of the lining pulled away. I had no clutch problems during my ownership, so who knows when it happened, as I would have thought that a chunk of clutch facing rattling around in there would have created some problems until it finished destructing.
So it goes.
BTW does anyone know how thick the facing should be on a fresh disc? Just curious.
Anyone had any luck with having a disc re-faced, as opposed to buying a re-manufactured one?
