Making the TÜV Man happy - Musterbrief?

I'm working to get my little S3 FHC on the road here in Germany & the TÜV man needs a bit more info. From searching, it looks like there was a Musterbrief floating around here a few years ago - does anyone still have a copy of that they could send me, or failing that, would someone be willing to share a copy of their Teil I (really only need technical data from right side of the Teil I)?

It's a '66 (pre-airflow).
